Health Coaching Programs
After Subscribing: Once you have selected a program, our office will contact you.
Insurance & Billing: Patients are billed directly for this program. The monthly fees you pay are a retainer for services. All programs are billed monthly upon enrollment and will be charged on the same date each month until paid in full. Your payment schedule will not follow your program schedule. At enrollment, DPC patients can choose to be billed monthy or annually to satisfy the 12-month minium committment. We may at our sole discretion work with you either by providing you super-bills & receipts or by assisting to submit a claim insurance for reimbursement. Each health insurance is different, please contact your insurance provider directly to inquire about your "Out Of Network" deductible and co-payments. Our billable services for this program include several physician visits, as well as chronic care management. If you have any questions, please call our offices. By starting our program you agree to the terms listed and give permission for medical services rendered to be billed to insurance for coverage. We do not participate in Medicare or Medicaid, and bills CANNOT be submitted to them for reimbursement. If you have any questions, please call your office. Please note, all Health Coaching & Medical Weight Loss Programs require a 2-month minimum with 30-days notice of cancellation.
State Licensure: Dr. Tro Kalayjian is licensed to practice Medicine in NY, NJ & CT, in these states he can order lab work, make diagnoses, & prescribe medications. If you are located outside of these states, sign up for our 16-week health coaching program. Please be advised that health coaching is NOT medical advice. Health coaching does not establish a patient-physician relationship. Our health coaches CAN provide education and support but they CANNOT make any medical diagnoses, prescribe medications or alter medical plans. Group Coaching: Initial Group Coaching Session must be completed prior to participating in Monthly Group Coaching Sessions. Group Coaching requires a 4-month minimum commitment and is billed monthly upon enrollment and will be automatically charged on the same date each month. The monthly fees you pay are a retainer for Group Coaching sessions only and do not establish a physician-patient relationship. Group Coaching is for educational purposes only and is not to be considered medical advice. Each session and/or Monthly payment of Group Coaching is nonrefundable, nontransferable, cannot be prorated and cannot be banked. All sessions are meant to be completed consecutively upon enrollment. Should you miss a Group Coaching session, you lose that session and it cannot be made-up.
Availabilty: Appointments are based off of availabilty. The office does its best to schedule all patients/clients as soon as possible.
